Appointment Information:

Please call 205-780-4330 to make or inquire about an appointment with our cardiologists.

We ask that you bring the following to your scheduled appointment:

  • Insurance card (or cards)
  • Driver’s license or Photo ID
  • Updated medication list (or actual medicine bottles)
  • Name and contact information for your Primary Care Provider

Insurance and Billing:

We make every effort to participate in most insurance programs. Co-pays are due at the time of service. You will also be responsible for deductibles and payment for non-covered services, so it is very important to confirm your insurance benefits with your insurance carrier prior to your appointment. If circumstances prevent you from being prompt in paying your bill, please contact one of our patient account representatives so that they may assist you in arranging a mutually acceptable plan of payment.

Emergencies:

If you have prolonged chest pain, any sign of a heart attack, or believe you are experiencing a true medical emergency, please go directly to the emergency room or call 911 for emergency help. The emergency room physician will examine you and then communicate with your cardiologist.

After you have received emergency medical attention, please call our office and let us know. During regular office hours, our staff will answer your questions and then give you instructions to follow. If the office is closed, please leave a detailed message with our after hours answering service; they will relay your message to the on call physician who will return your call if necessary.
